Hospice of the Golden Isles partners with several long-term care facilities to meet the needs of patients in their last months of life. The Hospice Social Worker and Chaplain work with the patient and family to provide emotional and spiritual support and guidance. Hospice Nurses collaborate with the long-term care physicians and staff in the management of pain and other symptoms and spend quality time with the patients and their families. The Hospice team offers education and assistance with the end-of-life processes. The Hospice Nurse also provides education to the facility’s nursing staff to help identify subtle signs unique to the terminally-ill and how to administer compassionate pain management and other symptoms effectively. Hospice Certified Nursing Assistants provide personal care to compliment aide services in the long term care facilities. The Hospice team advocates for the patient and the patient’s family regarding end-of-life care within the long-term care facility.
