OUR SERVICES

Under the care of an expert interdisciplinary team, patients of Hospice of the Golden Isles may receive the following good and /or services:

  • Medical Services under the supervision of a Medical Director - Our Medical Director is double board certified in Hospice and Palliative Medicine and Internal Medicine.
  • RN Case Management – Case managers work with the patient and their caregiver to develop customized plan-of-care.
  • On Call Skilled Nursing – Available 24 hours per day, 7-days per week, to assist patient and family members with pain and symptom management, education and medication consultation.
  • Certified Nursing Assistants – Assist with bathing, dressing, feeding and other activities of daily living.
  • Social Workers and Chaplains – Provide counseling, spiritual support, referrals to community and financial resources.
  • Prescription Drugs – Delivery and management of all medications that address palliative care and treatment related to a patient’s hospice diagnosis.
  • Medical Supplies – Provision of supplies such as catheters, IV’s wound care items as needed.
  • Durable Medical Equipment – Provision of durable Rx medical equipment such as oxygen, hospital beds, bedside commode, shower chair etc., as needed.
  • Trained Volunteers – Help relieve patients and families through offering companionship in a home or inpatient setting and/or brief stays in the home to provide caregivers with brief respite opportunities.
  • Bereavement Program – Offers community grief support through education, bereavement support groups, individual and family counseling for up to 13 months after a loss.
  • Therapies according to a patients plan-of-care

Patients of Hospice of the Golden Isles may receive one or more of the following types of care, which are all covered by the hospice benefit, according to standards set by Medicare.
